Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around New Castle, DE. This role offers a flexible schedule and travel reimbursement, enabling you to balance professional goals with personal commitments.

Qualified NPs will perform non-invasive assessments in patients' homes, review histories, and provide basic health education, while ensuring accurate documentation and compliance with safety standards.

Qualifications

• Master's degree in nursing or equivalent NP qualification.
• Active State NP license.
• Current national board certification.
• BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes and conduct comprehensive, non-invasive HRAs.
• Review medical history and document findings accurately.
• Provide basic patient education on wellness and preventive care.
• Ensure patient safety and documentation accuracy.
• Coordinate with scheduling to support timely visits.

Position Summary

Qualified Nurse Practitioners interested in this position will have the opportunity to interact with members in a more intimate setting. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements

• Master's Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
