Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around New Haven, CT. NPs will travel to patients’ homes and conduct comprehensive, non-invasive assessments in a setting that supports flexibility and community connection.

The role emphasizes assessment accuracy, documentation, and patient education, with training provided and travel reimbursement offered as part of competitive compensation.

Qualifications

• Master’s Degree in Nursing or commensurate NP experience required.
• Active State NP license and National Board Certification required.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to patients' homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients’ homes.

Nurse Practitioner Qualifications and Experience Requirements
