Hueman is seeking Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members around Middletown, CT. This opportunity offers flexible scheduling and the freedom to meet professional goals on your terms, while supporting communities with personalized care.

NPs performing HRAs do not prescribe in the home, focusing on assessment, education, and safety. Qualified candidates will hold an active CT NP license, national board certification, and BLS.

Qualifications

• Master’s degree in Nursing and CT NP license.
• National board certification and BLS.
• 12 months of experience as paid advanced clinical provider preferred.

Responsibilities

• Travel to patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

Nurse Practitioner Qualifications and Experience Requirements:

• Master’s Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
