Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Indianapolis, IN. This role offers flexible scheduling that fits your lifestyle, with a focus on meaningful community connections.

As an NP, you will travel to patients’ homes to perform non-invasive assessments, review medical history, and document findings. A current NP license, board certification, and BLS are required; prior paid advanced clinical

Qualifications

• Master's degree in nursing or equivalent.
• Active State NP license.
• National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

• Travel to patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately and timely.
• Conduct medical reconciliation for safety and compliance.
• Provide basic patient education on wellness and preventive care.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements

• Masters Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
