Position Overview

During visits lasting up to one hour, providers review health status, medical history, medication adherence, social environment, and other risks, providing insights to improve care quality and close care gaps with a single visit.

Responsibilities

• Conduct assessments including medical history, diagnosis, treatment, health education, and referrals.
• Collaborate with healthcare team members and family caregivers.
• Provide follow-up and patient education in various settings such as home visits and skilled facilities.

Position Details

• Type: PRN - Flexible hours
• Location: Home, SNF, community environments in Coudersport, PA
• Compensation: Earn from $2,000 to $10,000/month based on visits (pay of $100 per assessment)
• Hours: Days, evenings, nights, weekends, flexible scheduling
• Benefits: Non-benefited except 401(k) and voluntary benefits; mileage reimbursement included

Qualifications

• Master’s Degree or equivalent experience and NP licensure.
• Current RN and NP licensure with prescriptive authority.
• Board certification by AANP, ANCC, or AACCN in approved specialty.
• Current BLS, ACLS, or CPR certification.
• 1 year NP experience preferred; new grads encouraged.
• Strong computer skills and excellent communication skills.

Travel Requirements

• Travel to patient homes required; valid driver’s license and insurance needed.
