About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

Matrix Provider – What to Expect:

• Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other appropriate medical personnel.
• Collaborate with Primary Care Physician (PCP) on patient education and follow-up.
• Provide services in various venues including Home Visits, Skilled Facility Visits, and Telehealth Visits.

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN/Part Time - Flexible Nurse Practitioner

PAY: Earn from $2,300 up to $11,500 per month (based on an average of 20-100 completed visits per month, at $115 per assessment). Compensation varies with availability and assessment completion.

Location: Home, SNF, Telehealth, and other community environments in Whittier, California

Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling

Benefits: PRN/Part-Time providers are non-benefited except for 401(k) and limited voluntary benefits. Mileage reimbursement plan included.

Qualifications:

Must Haves:

• Master’s Degree required or equivalent experience with successful NP licensure.
• Current RN and NP licensure in the practice state, including prescriptive authority or ability to obtain it.
• Board certification by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.
• Comfortable and adaptable to change.

Travel Requirements:

• Ability to travel, valid driver’s license, and proof of adequate automobile insurance coverage.
• Travel required to meet patients at their locations.
