Join us as a Nurse Practitioner - PRN at HealthFair

Matrix Medical Network offers a broad range of clinical services and expertise that empower primary care providers and at-risk health plan members to better manage their health at home. Our network of clinicians breaks through traditional barriers by meeting members where they are, helping older adults and at-risk individuals improve their quality of care, achieve better health outcomes, and identify undiagnosed chronic conditions.

About the Position

During visits lasting up to one hour, you will review and observe a member’s health, medical history, medication adherence, social environment, and other risks. This comprehensive assessment aims to improve care quality and close care gaps efficiently.

Responsibilities

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, referrals, and documentation.
• Collaborate with physicians, case managers, social workers, family members, and other medical personnel.
• Coordinate with Primary Care Physicians on patient education and follow-up care.
• Provide services in various settings, including home visits and skilled facility visits.

Qualifications

• Master’s Degree or equivalent experience with NP licensure.
• Current RN and NP licensure, including prescriptive authority.
• Board certification by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with medical record software.
• Excellent communication skills and flexibility to adapt to change.

Additional Details

Type: PRN - Flexible schedule
Pay: From $1,800 to $9,000 per month based on completed assessments
Location: Lexington, KY and community environments
Hours: Days, evenings, nights, weekends, with flexible scheduling

Travel Requirements

• Travel to patient locations is required.
• Valid driver’s license and auto insurance are necessary.
