Concentra is seeking an Onsite Nurse Practitioner to join the onsite clinic team and deliver high-quality care under the direction of the Supervising physician or Director of Medical Operations. • Bachelor's degree from an accredited college/university or equivalent education and experience.
• Master’s degree in nursing or related health field from an accredited college or university.
• Graduate of an accredited Nurse Practitioner program.

Responsibilities

• Spends 100% of time in the onsite clinic providing direct patient care, leading by example, and creating an exceptional patient experience.
• Ensures day-to-day execution of Concentra's clinical model collaborating with healthcare team to drive outcomes.
• Monitors and improves onsite clinic metrics related to quality outcomes.
• Advanced Primary Care: chronic conditions, preventive care, screenings, vaccines, SDOH, wellness initiatives.
• Occupational Medicine: return-to-work measures, case duration, work duty status, and functional capacity.
• Provides direct patient care: history, exam, diagnosis, and treatment plans.
• Administers or orders diagnostic tests (x-ray, ECG, labs).
• Maintains patient records and paperwork with accuracy.
• Interprets diagnostic test results; performs procedures (injections, wound care).
• Counsels patients on treatment adherence and health maintenance.
• Listens to client needs and supports education and policy development.
• Contributes to education programs and ensures quality care.
• Delivers excellent customer service aligned with Orange Book values.

Job description

The Onsite Nurse Practitioner works with the Onsite Leadership Team at the assigned onsite clinic, and under the direction of the Supervising physician or Director of Medical Operations to implement the location’s medical policies, procedures, and programs ensuring the delivery of efficient, responsible and accountable healthcare of the highest quality to patients, businesses, and industry in accordance with Concentra's Best in Class practice.

Essential Duties and Responsibilities

• Spends 100% of time in the onsite clinic providing direct patient care, leading by example, and creating an exceptional patient experience.
• Ensures day-to-day execution of Concentra's clinical model collaborating with members of the healthcare team to drive optimal clinical outcomes.
• Accountable for monitoring, understanding, and effecting key onsite clinic metrics that relate to quality outcomes.
• Advanced Primary Care - chronic condition management, gaps in care, preventive health and age-appropriate screenings, vaccinations, SDOH, wellness initiatives, clinic utilization and patient satisfaction.
• Occupational Medicine - timely return to work measures after an injury on the job, case duration, lost time, work duty status, reduction in work-related illnesses and improved functional capacity.
• Provides direct patient care through compilation of the medical history, conduction of the physical exam, diagnosis, and the development of appropriate treatment plans.
• Administers or orders diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests.
• Maintains timely, accurate and detailed patient records and paperwork.
• Interprets diagnostic test results for deviations from normal.
• Performs therapeutic procedures, such as injections, immunizations, suturing and wound care, and managing infection.
• Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and health maintenance.
• Listens to and understands internal and external client needs.
• Assists in the development, implementation and revision of policies affecting medical practice judgment and quality of clinical care.
• Assists in the development and presentation of education and instructional programs.
• Provides excellent and compassionate customer service to all patients, clients and peers while incorporating Orange Book values.

Education/Credentials

Major:Nursing or related health field

Degree must be from an accredited college or university.

• Bachelor’s degree from an accredited college or university or equivalent education and experience
• Master’s degree in nursing or related health field from an accredited college or university
• Graduate of an accredited Nurse Practitioner program

Certifications and/or Licenses:

• Current licensed nurse practitioner in the state where employed and in accordance with state laws of practice
• Must maintain work state nurse practitioner licensure throughout the course of employment

Job-Related Experience

Customarily has at least the following experience:

3 years

• Customarily has at least three years of demonstrated experience in occupational medicine, urgent care or an emergency setting
