About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

Matrix Provider – What to Expect:

• Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education and follow-up
• Provide services in various venues including Home Visits and Skilled Facility Visits

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN - Flexible Nurse Practitioner

Pay: Earn from $2,000 up to $10,000 per month (based on 20-100 completed visits per month at $100 per assessment). Compensation varies with availability and assessment completion.

Location: Home, SNF, and other community environments in Mission, Texas

Hours: Days, evenings, nights, weekends, with flexible scheduling

Benefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits

Qualifications:

Must Haves:

• Master’s Degree required or equivalent experience with satisfactory NP licensure
• Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it
• Board certified by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum 1 year experience as a Nurse Practitioner
• Strong computer skills and familiarity with medical record software
• Excellent verbal and written communication skills
• Comfortable and flexible with frequent changes

Travel Requirements:

• Ability to travel, valid driver’s license, and proof of automobile insurance
• Travel required to meet patients where they live

Job: Clinical—Nurse Practitioner or NP. Full or Part-Time. Standard hours are 8 am to 5 pm, with travel, Monday through Friday. No on-call, no evening or weekend hours unless desired. This is a work-from-home opportunity with flexible hours and pay.
