Matrix Medical Network offers a broad range of clinical services and expertise to help primary care providers and at-risk health plan members manage their health at home. We focus on clinical assessment and care management, meeting members where they are to improve care quality, health outcomes, and detect undiagnosed chronic conditions.

Position Overview: During visits lasting up to one hour, providers review health status, medical history, medication adherence, social factors, and other risks, providing valuable insights and addressing care gaps through comprehensive assessments.

Responsibilities include:

• Conducting adult/geriatric assessments, including history, diagnosis, treatment, education, and referrals.
• Collaborating with healthcare team members and primary care physicians.
• Providing services in various settings such as home visits and skilled facilities.

Details:

• Type: PRN - Flexible Nurse Practitioner
• Pay: Earn from $2,000 to $10,000/month based on assessments completed.
• Location: Home, SNF, community environments in Littlestown, PA.
• Hours: Flexible days, evenings, nights, weekends.
• Benefits: Non-benefited except 401(k) and voluntary benefits; mileage reimbursement included.

Qualifications:

• Master's Degree or equivalent experience with NP licensure.
• Current RN and NP licensure with prescriptive authority.
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
• Current BLS, ACLS, or CPR certification.
• 1+ year NP experience preferred; new grads encouraged.
• Strong computer skills and excellent communication skills.

Travel: Must be able to travel to patient locations with valid driver's license and insurance.
