Position Overview

As a Nurse Practitioner conducting home risk assessments, you will review members' health, medical history, medications, and social environment during visits that can last up to an hour. This allows for comprehensive insight into their health and helps close care gaps efficiently.

Responsibilities

• Conduct adult/geriatric assessments, including history, diagnosis, treatment, education, and referrals.
• Collaborate with healthcare team members and primary care physicians.

Details

• Location: Home, SNF, community environments in Williamsport, PA.
• Flexible hours, including days, evenings, nights, weekends.
• Compensation: $2,000 - $10,000/month based on assessments completed.
• Benefits: PRN providers are non-benefited, with exceptions for 401(k) and voluntary benefits.

Travel is required to meet patients in their homes; valid driver’s license and insurance are necessary.
