About the Position:

During visits that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides comprehensive insight into a member’s overall health and well-being that is difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment aims to improve quality of care and address multiple care gaps through a single visit.

Responsibilities:

What to Expect:

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Collaborate with physicians, case managers, social workers, family members, caregivers, and other medical personnel as appropriate.
• Work with Primary Care Physicians on patient education and follow-up.
• Provide services in various venues, including home visits and skilled facility visits.

Position Details:

Type: PRN - Flexible - Nurse Practitioner

Pay: Earn from $1,800 up to $9,000 per month, based on 20-100 completed visits at $90 per assessment. Compensation varies with availability and assessment completion.

Location: Home, SNF, and community environments in Lewisburg, WV

Hours: Days, evenings, nights, weekends; flexible scheduling.

Benefits: PRN providers are non-benefited, except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan included.

Qualifications:

• Master’s Degree or equivalent experience with NP licensure.
• Current RN and NP licensure in the practicing state, including prescriptive authority or ability to obtain it.
• Board certification by AANP, ANCC, or AACCN in an approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with medical record software.
• Excellent communication skills with patients and colleagues.
• Flexibility and comfort with frequent changes.

Travel Requirements:

• Travel required to meet patients at their locations.
• Valid driver’s license, ability to drive, and proof of adequate automobile insurance.
