About the Position:

During visits lasting up to one hour, providers review health status, medical history, medication adherence, social environment, and risks. This comprehensive assessment enhances care quality and helps close care gaps efficiently.

• Conduct assessments for adults and geriatrics, including history, diagnosis, treatment, education, referrals, and documentation
• Collaborate with healthcare team members and family caregivers
• Coordinate with Primary Care Physicians for patient education and follow-up
• Provide services via home visits and skilled facility visits

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN - Flexible - Nurse Practitioner

PAY: Earn from $2,000 to $10,000/month based on 20-100 visits at $100 each. Compensation varies with availability and assessment completion.

Location: Home, SNF, and community settings in Hondo, Texas

Hours: Days, evenings, nights, weekends; flexible scheduling

Benefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits. Mileage reimbursement provided.

Qualifications:

• Master’s Degree or equivalent experience plus NP licensure
• Current RN and NP licensure with prescriptive authority in the state of practice
• Board certification by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum 1 year NP experience
• Strong computer skills and familiarity with medical record software
• Excellent communication skills
• Adaptability to change

Travel Requirements:

• Travel to patient locations required
• Valid driver’s license, ability to drive, and proof of insurance needed
