You will work with physicians and a multidisciplinary team to develop treatment plans, educate patients on prevention, and ensure accurate EMR documentation.

Qualifications

• Active Texas NP license and board certification.
• Experience in primary care or outpatient clinic.
• Strong clinical assessment and decision-making skills.
• Excellent communication and team collaboration.

Responsibilities

• Provide comprehensive patient care in an outpatient clinical setting.
• Perform patient assessments, physical examinations, and follow-up visits.
• Diagnose and manage acute and chronic medical conditions.
• Develop and implement individualized treatment plans.
• Prescribe medications and treatments as appropriate.
• Educate patients on preventive care, wellness, and disease management.
• Maintain accurate and timely EMR documentation.
• Collaborate with physicians and interdisciplinary care teams to ensure quality care.
