Neighborhood Health in Springfield, TN seeks a Nurse Practitioner to deliver high-quality primary care within our community health network. You will work with a team of clinicians to provide ambulatory and facility-based services, following established protocols with the Chief Clinical Officer.

The role emphasizes care access for underserved populations, teamwork, and continuous quality improvement, including integration of behavioral and dental health when appropriate.

Qualifications

• RN and MSN education required.
• Licensed/certified in Tennessee; BLS; continuing education as required.

Responsibilities

• Provide health care services to patients at Neighborhood Health centers.
• Participate in quality assurance and patient satisfaction activities.
• Support care coordination between patients, physicians and health teams.
• Utilize electronic health records to document care and achieve Meaningful Use standards.
• Contribute to community outreach and improving access to care for underserved populations.
