Qualifications

• Must hold California licensure as a Nurse Practitioner.
• Board certification or eligibility through ANCC or AANPCB is required or anticipated.
• Active DEA license is required.
• Current BLS certification is required.
• Bilingual English/Spanish is required.
• Prior primary care experience is preferred.
• Experience caring for medically complex patients is desired.
• Experience with managed care populations and quality-based programs is a plus.

Job description

Citrus Valley Medical Associates is seeking an experienced and compassionate Nurse Practitioner (NP) to join our Primary Care team.

We are looking for a provider who is passionate about delivering high-quality, patient-centered care and is comfortable managing a diverse adult patient population, including geriatric patients with complex acute and chronic medical conditions.

As part of our collaborative care team, you will work closely with physicians, fellow Advanced Practice Providers, clinical staff, and leadership to provide comprehensive care while supporting quality-driven healthcare initiatives.

As a Nurse Practitioner you will provide comprehensive primary care services, including:

• Evaluate and manage adult and geriatric patients
• Obtain comprehensive medical, social, and developmental histories
• Perform physical examinations
• Diagnose and manage acute and chronic illnesses
• Order and interpret appropriate laboratory and diagnostic studies
• Develop and manage individualized patient treatment plans
• Provide patient education, counseling, and preventive care guidance
• Manage medically complex and high-risk patients
• Consult with and refer patients to appropriate specialists when indicated
• Coordinate care with physicians, clinical staff, specialists, and other members of the healthcare team
• Exercise appropriate clinical judgment when establishing priorities of care, including triage situations
• Participate in quality improvement and quality-driven healthcare programs
• Provide case management and continuity of care in collaboration with the healthcare team
• Follow established medical group policies, procedures, managed care requirements, and applicable clinical guidelines

Required Qualifications

• Current California licensure as a Nurse Practitioner
• Board eligible or board certified through an applicable recognized certifying organization, including ANCC or AANPCB
• Active DEA license
• Current BLS certification
• Bilingual English/Spanish required
• Prior primary care experience required
• Experience managing medically complex patients
• Experience working with managed care populations and quality-based healthcare programs
• Strong clinical knowledge in the management of acute and chronic medical conditions
• Excellent interpersonal and communication skills
• Ability to communicate effectively with patients, physicians, Advanced Practice Providers, clinical staff, and administration

Preferred Qualifications

• NextGen EHR experience preferred

Hybrid Work Opportunity

This position may include a combination of on-site and remote work. Remote work opportunities will vary based on patient care responsibilities, operational needs, scheduling, and medical director approval. The provider must remain available to work on-site as required to meet patient care and clinic needs.
