Qualifications

• Bachelor’s Degree (R) required.
• Graduate of a Nurse Practitioner education program registered by the NYS Education Department (R).
• Master’s Degree (R) required.
• Licensed and currently registered as a Registered Nurse by NYS.
• Current New York State Registration as Nurse Practitioner.
• Current Board Certification as an NP from a NYS approved accrediting body.

Responsibilities

• Obtains and documents patient history per chief complaint or visit reason.
• Conducts relevant physical exams and evaluates patients, documenting findings.
• Diagnoses and treats disease and manages chronic illness.
• Prescribes medication in accordance with NYS and federal law.
• Orders lab, radiology and special tests; interprets results.
• Provides preventative health services, including education and counseling.
• Collaborates with other clinicians to ensure continuity of care.
• Performs other job-related duties as required.

Summary of Position

Nurse Practitioners provide medical services in collaboration with a physician, for patients in primary care as well as in medical and surgical specialty areas.

Principal Accountabilities

• Provides preventative health services, including health education and counseling.
  
• Collaborates with other clinicians and staff members to ensure continuity of care of patients.
  
• Performs other job-related duties as required.

Qualifications

Education, Training, Licenses, Certifications

• Bachelor’s Degree (R)

Relevant Work Experience, Knowledge, Skills, and Abilities

• 1-3 years of related experience (P)
  
• Experience working with diverse populations in an ambulatory healthcare setting. (P)
  
• BLS (R)
  
• ACLS strongly preferred
  
• Excellent verbal and written communication skills (R)
