Job summary

Diversity Group seeks a physician/NP/PA to provide comprehensive primary care to a veteran panel within a Patient Aligned Care Team. You will lead a team, coordinate with specialists, and ensure high-quality, patient-centered care.

The role involves history taking, exams, ordering labs, developing treatment plans for acute and chronic conditions, and thorough documentation. Telework is eligible; schedule is 8:00am–4:30pm, Monday–Friday.

Responsibilities

• Provide comprehensive primary care services to a defined panel of patients
• Lead a Patient Aligned Care Team (PACT) and coordinate care with the clinic
• Monitor PACT performance to ensure VA standards and quality of care
• Perform history taking, physical examination, and order labs
• Develop and implement diagnostic and treatment plans for acute and chronic conditions
• Document clinical care accurately in patient records
• Collaborate with a multidisciplinary team including social work, pharmacy, mental health, nutrition
• Lead patient care and administrative activities at the main hospital or another site if needed
• Maintain excellent communication with staff and patients; ensure outstanding customer service
• Work Monday–Friday from 8:00am to 4:30pm; Telework eligible

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The position calls for excellent interpersonal and leadership skills and offers input into projects pertaining to the development of the clinic
• Will provide comprehensive primary care services to a defined panel of patients
• Duties include history taking - physical examination - ordering laboratories and diagnostic studies - diagnosis and formulation of treatment plans for acute and chronic conditions within the scope of Medicine practice
• This provider (MD - DO - NP or PA) is the leader of a Patient Aligned Care Team (PACT) (physician - RN - LPN - and clerical associate) which provides direct care for Veterans enrolled in Primary Care
• This provider is assigned responsibility for a panel of patients at a specific location
• However - the provider may be asked on some occasions to assist in patient care or administrative duties at the main hospital or at another site
• Provides clinical examination - diagnosis - and treatment of acute and chronic diseases - preventive care - and coordination of care for a panel of primary care patients
• Appropriate documentation of patient care in clinical records is expected
• The physician refers to specialists when additional input and treatment outside of the scope of the physician's experience and training is required
• Major duties and responsibilities include: Responsible for monitoring PACT teamlet performance to assess adherence to VA standards - assure quality and effectiveness of care - and meet VA performance measures
• Outstanding customer service is expected
• Leads the team in assisting the patient advocate and administration in resolving patient concerns and inquiries
• Exercises medical decision-making and provides a broad range of diagnostic and therapeutic services for a specific group of patients
• Must have excellent communication skills and relations with staff and community and is able to elicit cooperation from others
• The primary care provider serves as a member of a larger multidisciplinary treatment team which includes social work - pharmacy - mental health - nutrition - and on occasion - other services
