Qualifications

• Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
• 1+ years of post-acute/SNF/LTC experience.
• Prior post-acute, long-term, and/or skilled nursing facility experience.

Responsibilities

• Provide high-quality care in collaboration with the attending physician.
• Manage patients with acute and chronic diseases in post-acute and long-term care settings.
