Qualifications

• Post-acute/SNF/LTC experience required.
• Board-certified geriatric, family, or adult NP per ANCC/ACHPN/AANP.
• Strong communication and collaboration with multidisciplinary teams.

Responsibilities

• Work with attending physician to manage patients with acute and chronic diseases.
• Collaborate with Director of Nursing, Social Workers, and Administrators to ensure high-quality care.
• Provide high-quality care while maintaining a compassionate patient-focused approach.
