The Clinician in a Post-Acute Care setting provides comprehensive evaluation, treatment, and care management services to patients transitioning from acute hospitalization to rehabilitation, skilled nursing, long-term care, or home-based care environments. This role focuses on improving functional outcomes, managing chronic and complex medical conditions, reducing hospital readmissions, and supporting safe transitions of care. The clinician collaborates with interdisciplinary teams, including physicians, nurses, therapists, case managers, and caregivers, to develop and implement individualized care plans that promote recovery, independence, and quality of life.
Key responsibilities include conducting patient assessments, monitoring clinical progress, coordinating care across settings, documenting treatment plans and outcomes, educating patients and families, and ensuring compliance with regulatory, quality, and safety standards. The clinician serves as an advocate for patients while contributing to organizational goals related to clinical excellence, patient satisfaction, and value-based care initiatives.
The BMH-owned or managed physician practices provide medical services in various specialties and offices including pediatrics, OB/GYN, cardiology, orthopedics, general surgery, family practice and internal medicine. The practices are staffed by physicians and other licensed providers who are supported by clinical staff (LPNs or medical assistants) and clerical staff (receptionists, transcriptionists, file clerks). The listed range represents the pay scale reasonably expected for a successful candidate based on factors such as relevant experience, education, internal equity, and market competitiveness.
