Job summary

Helena Valley Addiction Services in Helena, MT, is seeking a Physician Assistant/NP to evaluate opioid use disorder and develop medication treatment plans. This role conducts comprehensive medical histories, physicals, and annual evaluations, coordinating care in a multidisciplinary team.

The candidate will assess patients for MAT, monitor safety with EKGs and labs, and collaborate with primary care and SUD teams to ensure integrated care and effective treatment outcomes.

Qualifications

• Bachelor of Science in Nursing and graduation from an accredited Nurse Practitioner program.
• Active and unrestricted RN license in Montana.
• Active and unrestricted advanced registered nurse practitioner license for the state of Montana.
• Current DEA registration number, or ability to obtain one within the first 6 months of employment.
• A valid motor vehicle license and current auto insurance.
• One year of experience as a nurse practitioner in a community health agency, hospital, or provider’s office.

Job description

The Physician Assistant/ Nurse Practitioner’s primary responsibility is to evaluate patient’s opioid use disorder and develop medication treatment plans as appropriate. The provider will conduct thorough medical history and physical examinations on new patients, annual medical evaluations for established patients, make appropriate recommendations for follow-up of chronic medical conditions, and perform medication reviews for all enrolled patients. The Physician Assistant/ Nurse Practitioner’s will coordinate care in a multi-disciplinary team approach in regard to patient medical, emotion, and behavioral health conditions.

Major Responsibilities/ Activities:

• Evaluates and assess patients using various clinical tools such as vital signs, physical examinations, and COWS to determine type of MAT medication for treatment and dosage amount of MAT medication.
• Performs initial intake medical examinations to determine medical appropriateness and potential safety risks associated with administration of MAT medications, on site intakes as well as telehealth follow up appointments.
• Performs annual health history appointments on all established patients yearly.
• Meets with patients when there is a medical concern from fellow staff members that has the potential to be a safety risk for the patient to assess appropriateness for continued MAT.
• Assesses any and all patients who were recently discharged from the hospital to verify their last dose of medication related to MAT and determine if they meet medical criteria to continue with MAT
• Communicates with SUD staff and mental health staff regarding patient care to allow for collaborative, multi-disciplinary care
• Meets with patients who miss 4 or more days of MAT medication to assess for appropriate dosage amounts to allow for re-engagement in MAT services.
• Reviews EKG readings provided by medical assistants to monitor for prolonged QTc and/or arrythmias
• Reviews all intake laboratory results for potential health conditions or complications that require treatment and/or impact the patient’s ability to engage in MAT services
• Assists the Medical Director in developing referral resources from the health care community.
• Performs medication reviews by monitoring results from the Montana Pharmacy system every quarter for every patient assigned to the provider’s care
• Reviews medication report forms that show discrepancies in the total medication count
• Evaluates results from laboratory studies to determine patient’s appropriateness for split dosing of their MAT medications
• Educates patients on the medications they are taking by the prescribing provider and monitors for any potential complications related to polypharmacy
• Coordinates with the patient’s primary care provider regarding medication management to facilitate open communication and prevent medication errors and interactions
• Coordinates with the patient’s-controlled substance provider to discuss concerns related to medication mismanagement
• Participates in weekly case staffing with dispensing staff, SUDP staff, and medical staff
• Participates in Quality Improvement and Logic Models for program evaluation
• Complies with all record keeping and documentation regulations
• Maintains HIPAA and 42 C.F.R. Part 2 Confidentiality regulations
• Maintains compliance with all district required trainings
• May complete other duties and special projects as assigned

Job Qualifications:

Required Qualifications:

• Bachelor of Science degree in Nursing from Accredited School of Nursing plus graduation from an accredited Nurse Practitioner program
• One year of experience as a nurse practitioner in a community health agency, hospital, or provider’s office
• Active and unrestricted registered nurse license for the state of Montana
• Active and unrestricted advanced registered nurse practitioner license for the state of Montana
• Current Drug Enforcement Agency (DEA) registration number, or the ability to obtain one within the first 6 months of employment
• A valid motor vehicle license and current auto insurance

Preferred Qualifications:

• Master or Doctor of Nursing Practice degree from an accredited school of nursing with an accredited nurse practitioner program
• One year of experience as a nurse practitioner working in a behavioral health, public health, or addiction medicine setting. No previous experience with MAT or knowledge of the principles and techniques of treatment of co-occurring disorders is required, although it is preferred.
