Description
The Jersey Shore UMC per diem Stroke Service Nurse Practitioner or Physician Assistant performs the role utilizing the patient-centered coordinated care model demonstrated through competencies such as leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision making. Works collaboratively with other members of the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.
Responsibilities
- Provides medical assessments, treatments, and clearances for HMH applicants, team members, constituents, and external customers according to specific company profiles and regulatory standards.
- Performs clinical care professionally, serving as a customer advocate and role model.
- Ensures clinical practice complies with NJ Board of Medical Examiners regulations, Code of Ethics, NJ Department of Health requirements, and HMH policies and procedures.
- Develops and implements appropriate care plans.
- Facilitates patient teaching and support programs for patients and employers.
- Evaluates protocols and standards of care, supporting evidence-based practice protocols to achieve desired outcomes.
- Applies evidence and value-based practices to promote positive patient outcomes.
- Provides safe and effective medication management to support recovery from injuries or illnesses.
- Prescribes medications and recommends over-the-counter options as appropriate.
- Initiates laboratory tests, immunizations, and diagnostic procedures as regulated and within scope of practice.
- Demonstrates competent clinical skills with effective screening tests or procedures.
- Collaborates and consults effectively with physicians and the healthcare team.
- Maintains accurate and organized documentation of findings, recommendations, and patient education.
- Participates in continuing education, develops personal clinical expertise, and sets professional goals.
- Provides culturally appropriate education tailored to patient needs.
- Serves as a clinical resource across the continuum of care, evaluating complex patients and outcomes.
- Participates in policy updates based on current evidence and best practices.
- Acts as a resource for clinical, process, and system improvements to enhance care quality.
- Supports a collaborative and innovative work environment aligned with organizational values.
- Identifies ethical issues and coordinates interdisciplinary meetings to resolve them.
- Floats to different clinic locations as needed.
- Performs other duties and projects as assigned and adheres to organizational standards.
Qualifications
Required Education, Knowledge, Skills, and Abilities
- Graduate of an NLN/AACN accredited nursing program.
- Master's Degree in Nursing.
- Maintains clinical competencies through ongoing education and practice as mandated.
- Relevant experience in patient care settings or as regulated.
Preferred Qualifications
- Master's prepared Advanced Practice Nurse.
- Minimum of 3 years of professional experience.
- Strong organizational, computer, verbal, and written communication skills.
- Emotional intelligence, critical thinking, and interpersonal skills.
- Ability to adapt to changing priorities.
- Valid driver's license.
Licenses and Certifications
- NJ State Professional Registered Nurse License.
- Advanced Practice Nurse Certification with Master's Degree.
- AHA Basic Health Care Life Support (BLS) Certification.
Additional Certifications
- CPR certification (preferred).
Physician Assistant Specific Requirements
- Successful completion of an accredited physician assistant program by the AMA.
- Physician Assistant License.
- Basic Cardiac Life Support (BCLS) Certification.
