Job Summary

The Nurse Practitioner/Physician Assistant Primary Care SNF/ALF will provide primary care services to patients in skilled nursing facilities and assisted living facilities. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to provide high-quality care to patients.

Responsibilities

• Conduct comprehensive assessments of patients' health status
• Develop and implement treatment plans
• Order and interpret diagnostic tests
• Prescribe medications and other treatments
• Provide patient education and counseling
• Collaborate with physicians and other healthcare professionals to provide coordinated care
• Maintain accurate and complete medical records
• Participate in quality improvement initiatives
• Adhere to all applicable laws, regulations, and standards of practice

Qualifications

• Master's degree in nursing or physician assistant studies
