Eventus WholeHealth is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive primary care in skilled nursing and senior living communities. This role includes routine rounding, proactive chronic disease management, and coordination with interdisciplinary teams to deliver preventive, high‑quality care.

This part‑time position is three weekdays per week, focusing on resident‑centered care, strong communication with residents and families, and collaboration with facility

As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will provide comprehensive primary care services within skilled nursing and senior living communities. This role includes routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Qualifications:

• Current NP or PA licensure in the state of practice
• Previous professional patient care experience
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient‑centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams
