Job summary

Bluestone Physician Services is seeking a Part-Time Nurse Practitioner or Physician Assistant to deliver on-site primary care in Assisted Living and Memory Care communities around Plymouth, MN. You will manage a panel of about 100 patients, typically 12-16 per day, with schedule flexibility across approximately 2.5 days per week, Monday-Thursday.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification as required by work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver’s license; local travel is required.

Position Overview:
