Eventus WholeHealth is seeking an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care within skilled nursing and senior living communities. You will perform routine rounding and manage chronic diseases, coordinating care with interdisciplinary teams and facility partners to improve outcomes.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Excellent patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams
