Primary Responsibilities
  • Manage chronic illnesses in an elderly population with multimorbid conditions, focusing on wellness, reducing hospitalizations, and improving care quality and satisfaction.
  • Document care efficiently, focusing on closing HEDIS quality gaps and recapturing HCCs.
  • Conduct comprehensive assessments for new patients, including functional, social, safety, cognitive, and mental health evaluations.
  • Triage, diagnose, and treat patients in-home using available equipment and medications.
  • Order and interpret tests such as x-rays, labs, and EKGs within your scope of practice.
  • Initiate treatment at the patient's home.
  • Collaborate with primary care physicians and specialists as needed.

Required Qualifications
  • Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA-C) program
  • Current unrestricted NP or PA license in the state
  • Valid state driver’s license
  • Reliable transportation for patient visits; compliance with Landmark Motor Vehicle Safety policy required if driving a vehicle
Preferred Qualifications
  • At least 2 years of clinical experience as an NP or PA in IM, Geriatrics, Primary Care, ED, Urgent Care, or similar settings (highly preferred)

Note: Employees must be active in their role for at least 12 months from hire/transfer. Leaving the role before 36 months may affect student loan repayment eligibility. Full details are provided during the hiring process.
