Job summary

Clinical Renal Associates (CRAL), part of Panoramic Health, is expanding its APP team in the Greater Philadelphia region. The role focuses on nephrology care for patients with CKD and ESRD, across outpatient, inpatient, and dialysis settings.

You’ll work with nephrologists and a multidisciplinary team, receiving training and mentorship to build specialized nephrology expertise and improve long-term patient outcomes.

Qualifications

  • Graduate of an accredited NP or PA program.
  • Active, unrestricted license in the state of practice.
  • National board certification appropriate to the role.
  • Nephrology, dialysis, renal, hospital medicine, or critical care experience is valued but not required.

Responsibilities

  • Evaluate and manage patients with chronic kidney disease and related conditions.
  • Perform comprehensive histories and physical examinations and develop individualized treatment plans.
  • Order and interpret laboratory testing and diagnostic studies.
  • Manage medications and coordinate referrals and consultations.
  • Provide education focused on kidney health, disease progression, treatment options, and prevention.
  • Help patients become active participants in their care and treatment decisions.
  • Manage hemodialysis and/or peritoneal dialysis patients in collaboration with the nephrology team.
  • Coordinate continuity of care between dialysis units, hospitals, clinics, and other settings.

This is an opportunity to practice across a diverse nephrology environment, with exposure to outpatient clinics, hospitals, and dialysis settings throughout the Philadelphia region. You’ll work closely with nephrologists and a multidisciplinary care team while developing meaningful, longitudinal relationships with patients.

What You’ll Do

As an APP with Clinical Renal Associates, you’ll play an important role in managing patients throughout the continuum of kidney disease.

  • Evaluate and manage patients with chronic kidney disease and related conditions.
  • Perform comprehensive histories and physical examinations and develop individualized treatment plans.
  • Order and interpret laboratory testing and diagnostic studies.
  • Manage medications and coordinate referrals and consultations.
  • Provide education focused on kidney health, disease progression, treatment options, and prevention.
  • Help patients become active participants in their care and treatment decisions.

Dialysis Care

  • Manage hemodialysis and/or peritoneal dialysis patients in collaboration with the nephrology team.
  • Assess and manage complications associated with ESRD, including anemia, hypertension, bone and mineral disorders, fluid/electrolyte imbalances, and vascular access issues.
  • Monitor treatment response and adjust plans of care as appropriate.
  • Coordinate continuity of care between dialysis units, hospitals, clinics, and other healthcare settings.
  • Provide patient and family education and serve as a clinical resource to dialysis staff.
  • Perform admission histories and physical examinations and participate in inpatient management.
  • Evaluate patients with acute and chronic kidney disease and develop appropriate plans of care.
  • Participate in discharge planning and coordinate follow-up care.
  • Educate patients and families regarding treatment options and expected outcomes.
  • Help ensure continuity of care between the hospital, dialysis setting, and outpatient clinic.
  • Starting base salary: $110,000+, negotiable based on experience
  • Up to $20,000 productivity bonus opportunity
  • 5 CME days annually
  • 4 weeks of PTO

What We're Looking For
  • Nurse Practitioner or Physician Assistant with an active, unrestricted license in the state of practice
  • Graduate of an accredited NP or PA program
  • National board certification appropriate to the role
  • Strong clinical assessment, diagnostic, treatment planning, and patient management skills
  • Excellent communication and collaboration skills
  • Commitment to patient-centered care and clinical excellence
  • Nephrology, dialysis, renal, hospital medicine, or critical care experience is highly valued—but not required
