Relive Health Great Neck is seeking a Nurse Practitioner or Physician Assistant with expertise in hormone optimization, peptide therapy, and medical weight loss to provide in-person care in Great Neck, NY. Qualifications

• Active and unrestricted New York NP or PA license.
• Direct clinical experience with HRT/TRT, peptides, GLP-1s/medical weight loss, functional medicine, or longevity medicine.

Responsibilities

• Conduct patient consultations, assessments, and follow-up visits.
• Review and interpret laboratory testing.
• Develop individualized treatment plans and manage HRT/TRT.
• Manage medical weight-loss patients, including GLP-1 therapies.
• Evaluate and manage peptide therapy protocols.
• Monitor progress and adjust treatment plans as needed.
• Educate patients on treatments, risks, and benefits.
• Maintain accurate EHR documentation.
• Collaborate with RNs, medical assistants, and other providers.
• Follow HIPAA, safety and compliance requirements.

Nurse Practitioner / Physician Assistant – Hormone, Peptide & Functional Medicine

Location: Great Neck, NY – In Person

Compensation: $80–$100/hour, based on experience

Schedule: Part-Time, with opportunity to grow

Aesthetic Injectable Experience: A plus, but not required

Qualifications

• Active and unrestricted New York NP or PA license
• Direct clinical experience with HRT/TRT, peptides, GLP-1s/medical weight loss, functional medicine, or longevity medicine
• Comfortable reviewing bloodwork and developing treatment plans
• Strong patient communication and education skills
• Ability to work independently and collaboratively
• Aesthetic injectable experience is a plus, but not required
• IV therapy or regenerative medicine experience is a plus
