Relive Health Great Neck is seeking a Nurse Practitioner or Physician Assistant with expertise in hormone optimization, peptide therapy, and medical weight loss to provide in-person care in Great Neck, NY. This role emphasizes hands-on clinical experience and collaboration within a wellness-focused practice.

Qualifications

• Active and unrestricted New York NP or PA license.
• Direct clinical experience with HRT/TRT, peptides, GLP-1s/medical weight loss, functional medicine, or longevity medicine.

Responsibilities

• Conduct patient consultations, assessments, and follow-up visits.
• Review and interpret laboratory testing.
• Develop individualized treatment plans and manage HRT/TRT.
• Manage medical weight-loss patients, including GLP-1 therapies.
• Evaluate and manage peptide therapy protocols.
• Monitor progress and adjust treatment plans as needed.
• Educate patients on treatments, risks, and benefits.
• Maintain accurate EHR documentation.
• Collaborate with RNs, medical assistants, and other providers.
• Follow HIPAA, safety and compliance requirements.
