Daviess Community Hospital in Indiana is seeking a Nurse Practitioner or Physician Assistant to develop and implement treatment plans for acute and chronic diseases. You will educate patients on prevention and healthy lifestyles, and provide age-appropriate care across the life span.

The role requires active Indiana NP/PA licensure, a current BLS certificate, and EMR documentation proficiency. You will collaborate with care teams, address diverse patient needs, and adjust plans as patients

Qualificações

• Must have a current Indiana NP or PA license (DEA, NPI, CSR).
• Current BLS certification. ACLS preferred.
• Completion of required CME's.
• Computer skills for documentation of patients' charts in an EMR.
• Ability to communicate in English; both verbally and in writing.
• Additional languages beneficial, but not required.
• Demonstrates the ability to be flexible, organized and function under stressful situations.
• Must be able to multi-task as well as being a team player.

• Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family.
• Revises plan of care as indicated by the patients' response to treatment.
• Assists patients in managing chronic conditions, treating illnesses and helping patients make educated choices about their lifestyle, health and well-being.
