Responsibilities:
Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment
Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
Assists with the provision of care in accordance with facility, state, and federal regulations
Monitors the effectiveness of interventions
Maintains relationships with all appropriate contacts in facilities
Educates family members, clients, and prospective accounts regarding services and programs
Assists with the development and maintenance of physician relations, as assigned
Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
Other Duties as assigned by Medical Director
Qualifications:
Current Nurse Practitioner/ Physician Assistant License
Degree from an accredited Nurse Practitioner/ Physician Assistant Program
ACLS
Current infection control certification
Strong clinical collaboration with physicians
Proficient computer skills
PACS and EMR experience preferred
Committed to professional growth and learning
Not listed on the OIG exclusion list
Preferred Experience:
Vascular Care: Venous Insufficiency, Peripheral Arterial Disease
Image Guided Injectables
Advanced Geriatric
Skilled Nursing Facilities, Assisted Living Facilities
