Job summary

Qualifications

Responsibilities

• Provide patient care in a Family Practice setting.
• Utilize EPIC EMR for documentation and care coordination.
• Collaborate with nurses, case managers, and behavioral health staff.
• Contribute to a high-performing team with a patient-first approach.
