San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals aged 55 or older living with chronic illnesses or disabilities who need coordinated medical care to maintain independence at home and in the community. The healthcare teams at St. Paul’s PACE provide coordinated medical, social, and home care services, enabling individuals to manage their medical needs effectively.

We are seeking Full-Time Temporary Physician Assistants or Nurse Practitioners! The position is expected to last 6 months, with a possibility of extension. Typically, this is a Monday - Friday role.

Job Summary

Under the clinical supervision and direction of the Medical Director, provides primary care to participants. Performs physical assessments, develops and implements care plans, evaluates physical complaints, and provides treatment. Offers education to participants and caregivers. Collaborates as part of the Interdisciplinary Team, demonstrating the necessary skills to care for frail elder participants according to PACE standards.

Qualifications and Requirements
  • Education: Graduate of an accredited physician assistant or nurse practitioner program; licensed in California; certified by a recognized national certifying organization.
  • Experience: At least one year working with the elderly or frail populations; two years preferred. Relevant training or experience in the assigned role.
Additional Requirements
  • Valid California Physician Assistant or Nurse Practitioner license
  • Degree from an NCCPA-certified program
  • DEA Number
Skills and Knowledge
  • Knowledge of home and community-based care for the elderly and disabled
  • Understanding of PACE regulations (desired)
  • Knowledge of physical, mental, and social needs of frail older adults
  • Effective communication skills
  • Ability to work within an interdisciplinary team
