Job summary

Monogram Health seeks compassionate Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions, including diabetes, CKD, heart failure, COPD, and mental health needs.

The role operates within an integrated multidisciplinary team, providing continuity of care in patients' homes and via telehealth, in a value-based, patient-centered care model that emphasizes quality outcomes.

Qualifications

• Active and unrestricted RN or NP license, or PA license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate, or eligibility to obtain.
• Minimum 2 years of direct patient care experience.
• Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance.
• Knowledge of federal/state regulations (HIPAA, OSHA).
• Experience with EHR systems (Athena, Salesforce) preferred.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home to reduce hospitalizations and improve quality of life.
• Perform comprehensive assessments, including initial evaluations, wellness exams, follow-ups, and post-discharge visits.
• Educate patients and caregivers on disease management, preventive care, medications, and diagnostics.
• Participate in recurring virtual integrated care team meetings.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and promptly.

• Previousexperience conducting annual wellness or comprehensive visits preferred
• Prior experience with EHR systems (Athena, Salesforce) preferred
• Background in managed care, population health, or value‑based carepreferred
