This is an excellent opportunity to advance your nursing career as a CCM Nurse Practitioner/Physician Assistant on a per diem basis, providing periodic or intermittent care to Optum members.

Primary Responsibilities:
  1. Primary Care Delivery
    • Deliver cost-effective, quality care to assigned members.
    • Manage medical and behavioral, chronic and acute conditions, collaborating with physicians or specialists.
    • Perform assessments, document findings, and ensure compliance with CMS regulations.
    • Ensure diagnoses are accurately coded and documented.
    • Address all quality elements and conduct medication reviews.
    • Facilitate member care plans with involved parties.
    • Evaluate and revise care plans as needed.
    • Follow practice guidelines established by CCM.
    • Complete mandatory training and travel between care sites is required.
  • Care Coordination
    • Understand and communicate benefits, policies, and procedures.
    • Assess the necessity of ancillary services and coordinate care transitions.
    • Monitor member needs and adjust care plans accordingly.
    • Review treatment responses and optimize care plans for cost-effectiveness.
  • Program Enhancement Behaviors
    • Communicate effectively with all stakeholders.
    • Promote CCM program growth through stakeholder partnerships.
    • Innovate and operate independently.
    • Manage scheduling proactively and participate in quality initiatives.

    Required Qualifications:
    • Certified Nurse Practitioner through a national board.
    • Graduate of an accredited MSN program, board certified (AANP or ANCC), with preferred certifications like ANP, FNP, or GNP.
    • Active and unrestricted state license.
    • Current DEA licensure or ability to obtain.
    • Reliable transportation and ability to lift 30 pounds.
    • Availability to work at least 24 hours/month, including off-hours, not exceeding 960 hours/year.
    • Ability to gain a collaborative practice agreement if required.
    Preferred Qualifications:
    • 1+ years of post-grad experience in Long Term Care.
    • Knowledge of Geriatrics, Chronic Illness, and End-of-Life discussions.
    • Proficient in electronic medical records and developing positive relationships.
    • Adaptability to change.

    The hourly rate ranges from $43.17 to $84.95, based on various factors including experience and location.
