Harlem Hospital Center has been a bedrock of the Harlem community since 1887. Responsibilities
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Duties & Responsibilities

Purpose of Position:

Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive primary care services in a health care setting. Ensures patient and family centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care.

Essential Duties and Responsibilities:

  • Provides continuous comprehensive primary care medical and nursing care, including preventive and supportive health care services to patients and families.
  • Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
  • Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.
  • Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
  • Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.
  • Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
  • Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs.
  • Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
  • Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
  • Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.
  • Provides education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.
  • Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention and health promotion, records and maintains a system of relevant documentation.
  • Performs other related duties, as directed.
Department Preferences

Additional Preferences:

Strongly prefer National Board Certification in Women's, Adult or Family Health Nurse Practitioner (NP) Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners Certification Board (AANPCB).

Departmental Preferences:

Two clinical years experience as Women's, Adult or Family Health Nurse Practitioner (NP) preferred.

In addition to base compensation, you may also receive the following based on the Collectively Bargained Agreement:

  • A. An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:
    • 1. Bachelor of Science in Nursing (BSN) or Bachelor in allied health discipline = $900.
    • 2. Master of Science in Nursing (MSN) or Master in allied health discipline = $1,200.
    • 3. Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500.
  • B. An annual certification differential of $1878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.
  • C. An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
