Responsibilities
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
- Current certificate as a Nurse Practitioner issued by the NYSED.
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Duties & Responsibilities
Purpose of Position:
Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive primary care services in a health care setting. Ensures patient- and family-centered quality care to improve outcomes and patient experience in accordance with applicable policies and regulations.
Essential Duties and Responsibilities:
- Provides continuous comprehensive primary care medical and nursing care, including preventive and supportive health services to patients and families.
- Manages health problems and coordinates health care for various patient populations; identifies illnesses and implements therapeutic and corrective measures.
- Differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician intervention.
- Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and other health care professionals as necessary.
- Evaluates and interprets patient data to plan and implement appropriate nursing interventions.
- Consults with physicians and other practitioners to determine treatment decisions that support improved outcomes and patient satisfaction.
- Formulates differential diagnosis and care plans based on medical history, physical exams, and nursing assessments.
- Develops, implements, and evaluates patient care plans, adjusting them as per clinical standards to achieve health goals.
- Orders, conducts, and interprets diagnostic and laboratory tests; prescribes appropriate pharmacological and non-pharmacological interventions.
- Manages patient session schedules, ensuring availability and continuity of care and optimal resource utilization.
- Provides education, guidance, and direction to nursing staff and other team members to ensure quality and safe care.
- Educates and counsels patients and families regarding health status, treatment, medication management, disease prevention, and health promotion; maintains documentation.
- Performs other related duties as directed.
Department Preferences
- Prefer Board Certification in Women’s Health or Family Nurse Practitioner.
- Prior experience in antenatal care and LARC insertion/removal preferred.
