Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
  • Current certificate as a Nurse Practitioner issued by the NYSED.
  • Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Duties and Responsibilities
Rikers Island Nurse Practitioner

Step into a pivotal role within one of the nation’s largest jail systems, where we handle approximately 40,000 admissions each year and maintain an average daily population of 6,500 patients. CHS sets the standard for outstanding medical and mental health care in correctional medicine, leading the field with a population‑health, human‑rights, and evidence‑based approach.

Position Overview

Under the supervision of the Site Medical Director, Nurse Practitioners will provide comprehensive, compassionate, and thoughtful care to patients with complex chronic diseases within the New York City jail system. The role involves collaboration with physician leadership to provide general primary care, including history taking, physical exams, diagnosis, and management of acute and chronic illnesses, as well as consult coordination.

  • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.
  • Complete comprehensive histories and physicals on all new admissions, including documentation of problem list, diagnosis, orders (e.g., labs, imaging, referrals) and appropriate medication orders.
  • Evaluate patients requesting sick call, schedule follow‑ups, and update medication orders. Update problem lists and reconcile patient orders at all visits.
  • Implement care plans using protocols approved by the Medical Director and/or treatment plans reflecting current standards of care.
  • Request radiology exams, lab tests, EKGs when clinically indicated and interpret results in consultation with supervisors as appropriate.
  • Review specialty consults and hospital returns to ensure standards of care are met and consultant recommendations are implemented.
  • Perform chart reviews and summaries for patients transferring facilities, updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.
  • Generate special needs referrals and documentation for patients with disabilities, dietary restrictions, heat sensitivity, or other relevant flags.
  • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different decisions; sign and document patient refusals of care.
  • Ensure all progress notes and orders are signed before the end of the shift.
  • Respond to emergencies in a timely and professional manner.
  • Notify the staff MD and Urgicare about 3‑hour runs and EMS activation.
  • Complete special housing rounds when assigned.
  • Maintain knowledge of quality of care and population health indicators; take appropriate action to meet or exceed standards.
  • Maintain clinical competency by participating in all CME and CHS training and in‑service requirements.
  • Adhere to CHS policies and procedures; maintain required credentials and Occupational Health Services requirements.
  • Maintain and attend scheduled shifts with punctuality and timely notification of absences.
  • Complete tasks delegated by a Site Medical Director or other supervising clinical team member.
Department Preferences
  • Strong preference for National Board Certification in Family Health Nurse Practitioner (NP) by the American Nurses Credentialing Center (ANCC).
  • Preference for two clinical years’ experience as a Family Health Nurse Practitioner (NP).
Benefits and Compensation
  • Base Salary: $147,264
  • Annual Education Differential:
    • Bachelors in allied health discipline: $900
    • Masters in allied health discipline: $1,200
    • Bachelors of Science in Nursing (BSN): $1,800
    • Masters of Science in Nursing (MSN): $2,000
    • Doctorate in Education, Nursing Practice or PhD in Nursing (DNP): $2,500
  • Annual Certification Differential:
    • Behavioral Health, Emergency, Surgical Services, Critical Care: $3,000
    • Med-Surgical/Dialysis, Correctional Health Professional (if assigned to Correctional Health): $2,000
    • All other nationally recognized certifications: $1,878
  • Annual Nursing Experience Differential: $987 to $8,190 for up to 10 years of experience in a Staff Nurse role.
