Job Opportunity: Advanced Practice Clinician - Telephonic Care

$10,000 Sign-on Bonus for External Candidates

Work Schedule: Nights, Weekends, Rotating Holidays with telephonic after-hours calls

Role Overview:

The Home and Community Care (HCC) program offers longitudinal, integrated care. The National On-Call APC provides telephonic care and guidance during evenings, weekends, and holidays to a designated group of enrollees, supporting their medical needs remotely.

This remote role involves providing virtual after-hours care primarily via telephone, offering a fulfilling opportunity to support the elderly, with flexible work-life balance. Optum is a leader in APC employment and offers substantial career development.

Scheduling Details:
  1. This is a full-time, remote position with variable shifts including weeknights, weekends, and holidays.
  2. Coverage times include:
    1. Weeknights from 5pm-8am local time
    2. Weekend coverage from Friday 5pm to Monday 8am
    3. Holiday coverage from 5pm of the last business day to 8am of the next business day
  3. Shift options include 5PM-2AM or 11PM-8AM for weekdays, and 11AM-11PM or 1AM-1PM for weekends.
  4. Holiday schedules are planned in advance.
Preferred Experience:

Recent experience in high-acuity geriatric care (e.g., SNF, LTACH, Hospitalist, ICU, Emergency) is preferred.

Responsibilities:
  • Handle approximately 4 after-hours calls per hour from facility staff.
  • Accurately document notes live during each case.
  • Provide acute care via EMR, managing medical and behavioral conditions in collaboration with physicians or specialists.
  • Monitor member needs, review orders, and adjust care plans as needed.
  • Communicate effectively with all parties involved, including physicians, members, and facility staff.
Qualifications:
  • Active OH or WI NP license or ability to obtain by start date, with no unresolved disciplinary actions.
  • Active ANCC or AANP certification in relevant specialties or ability to obtain.
  • Current DEA license or ability to obtain.
  • Availability for weekend, weekday, and holiday shifts.
Preferred Qualifications:
  • 2+ years of postgrad experience in Long Term Care.
  • Knowledge of Geriatrics, Chronic Illness, and End-of-Life discussions.
  • Proficient in electronic medical records documentation.

The annual salary ranges from $104,500 to $156,000, based on various factors including experience and location. Benefits include comprehensive health coverage, incentive programs, stock purchase options, and 401k contributions.
