Position Overview: Optum Home & Community Care, part of UnitedHealth Group, is innovating in healthcare by providing holistic, accessible care. As a Senior Community Care (SCC) team member, you'll support patients at home, enhancing access and quality of care.

This remote, telephonic role involves providing after-hours care to aging residents in various settings, offering autonomy and work-life balance. Optum is committed to career development and transforming care delivery.

Scheduling

• This is a full-time, remote position with variable shifts on nights, weekends, and holidays.
• Coverage includes weeknights from 5pm to 8am, weekends from Friday 5pm to Monday 8am, and holidays from 5pm to 8am.
• Shifts are typically 5PM-2AM or 11PM-8AM on weekdays, and 11AM-11PM or 1AM-1PM on weekends.

Experience & Skills:

• Recent experience in high-acuity geriatric care preferred.
• Excellent typing skills for live note entry.
• Active NY license required; flexibility to work remotely.

Primary Responsibilities:

• Handle calls to coordinate care across facilities and providers.
• Use EMR proficiently for acute care documentation.
• Manage medical and behavioral conditions, collaborating with physicians.
• Document encounters accurately to support diagnoses.
• Participate in mandatory training.
• Monitor member needs, adjust care plans, review orders, and facilitate effective communication.

Qualifications:

• Active NY NP license or ability to obtain by start date, with no disciplinary restrictions.
• Active ANCC or AANP certification in relevant specialties or ability to obtain it.
• Current DEA license or ability to obtain.
• Availability for weekends, weekdays, and holidays.

Preferred Qualifications:

• 2+ years of postgrad experience in Long Term Care.
• Knowledge of geriatrics, chronic illness, and end-of-life discussions.
• Proficient in medical documentation and electronic records.
