• Full-Time (Weekdays | 8:00 AM–2:00 PM - may vary based on the facility) $100,000–$120,000 annually (DOE) Based on experience, clinical autonomy, and caseload expectations.

Responsibilities – Skilled Nursing Facility Provider

• Provide assessment, diagnosis, and management of acute and chronic conditions
• Conduct regular patient rounding and timely evaluations
• Order and interpret labs, diagnostics, and imaging
• Prescribe and manage medications in compliance with regulations
• Maintain accurate, timely documentation in the EMR
• Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams
• Respond to change-in-condition and urgent care needs
• Support advance care planning and end-of-life discussions
• Participate in quality assurance and regulatory readiness efforts
