Job summary

Delaware Valley Community Health in Philadelphia, PA, is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive medical services across 9 health centers. This position supports professional patient care, requires travel within the DVCH system, and offers a Monday through Friday schedule with full- or part-time options.

The ideal candidate will be a PA or NP licensed in Pennsylvania, with Adult/Family Practice experience and strong English–Spanish communication as a plus.

Qualifications

• Licensed NP or PA in Pennsylvania is required.
• Experience in Adult/Family Practice preferred.
• Excellent interpersonal and written and oral communication skills required.
• Bilingual English/Spanish preferred.

Responsibilities

• Act as an advanced care practitioner and assist in coordinating all aspects of patient care.
• Render professional medical services to the patient population.
• Complete documentation of patient visits in the patient chart and administrative reporting as assigned by management.
• Provide health and wellness advice to patients or caregivers.

What you will do:

This position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc. This position augments the Physician's data gathering abilities and assists in reaching decisions and instituting care plans for our patients. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management. This role will require travel to and from the 9 health centers operated by DVCH.

Duties include but are not limited to:

• Act as an advanced care practitioner and assist in coordinating all aspects of patient care.
• Responsible for rendering professional medical services to the patient population.
• Accountable for the complete documentation of patient visits in the patient chart and administrative reporting activities assigned by management
• Provide health and wellness advice to patients or caregivers.
• Completes other duties as assigned.

Qualifications, Education, and Knowledge:
