Job summary

American Physiatry seeks a compassionate Nurse Practitioner to join our in-person care team in North Carolina. You will assess patients, develop rehabilitation-focused plans, and coordinate with physicians and therapists to optimize function and pain control.

Qualifications

  • Prompt, accurate clinical documentation in accordance with policy.

Responsibilities

  • Conduct comprehensive patient assessments under supervision of a Physiatrist.
  • Develop and implement individualized care plans focused on function and rehab.
  • Collaborate with the team on medications and therapy recommendations.
  • Educate patients and families on treatment plans, health maintenance and prevention.
  • Monitor progress and adjust care plans to achieve optimal outcomes.

Key Responsibilities
  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
  • Educate patients and their families on treatment plans, health maintenance, and preventive care.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
Qualifications
  • Licensed Nurse Practitioner with a valid North Carolina certification.
  • Experience working independently while effectively managing your own caseload.
  • Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
