Job summary

American Physiatry is seeking a compassionate Nurse Practitioner to join our team in North Carolina. This part-time, in-person role focuses on pain control, function, and rehabilitation within skilled nursing facilities, in close collaboration with physiatrists and physical therapy staff.

You will conduct patient assessments, develop individualized care plans, and educate families on treatment and preventive care.

Qualifications

  • Licensed Nurse Practitioner with a valid North Carolina certification.
  • Experience working independently while effectively managing your own caseload.
  • Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.

Responsibilities

  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
  • Educate patients and their families on treatment plans, health maintenance, and preventive care.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
