Novenza Health in Kokomo, IN is seeking dedicated Nurse Practitioners to join our Post-Acute LTC team. You will assess, diagnose and manage patient care in nursing home and assisted living settings, collaborating with physicians to improve residents' quality of life.

Qualificações

• Current Nurse Practitioner license in Indiana with relevant certifications in adult, geriatrics or family practice.
• Experience in long-term care settings preferred but not required.
• Proficiency in EMR systems Ethizo, GEHRIMED, Matrix, PCC.
• Knowledge of ICD-9/ICD-10 coding.
• Background in critical care or hospital medicine is a plus; trauma center experience desirable.
• Strong communication skills and ability to collaborate within a multidisciplinary team.

Job Summary

Requirements

• Current Nurse Practitioner license in the state of practice with relevant certifications in adult, geriatrics or family practice.
• Experience in long-term care settings such as nursing homes or assisted living facilities is preferred but not necessary.
• Proficiency in EMR systems (Ethizo, GEHRIMED, Matrix, PCC ) for effective medical documentation.
• Knowledge of ICD-9/ICD-10 coding.
• Background in critical care or hospital medicine is a plus; experience in Level I/II trauma centers is desirable.
• Strong communication skills with the ability to work collaboratively within a multidisciplinary team environment.
