Novenza-Health in Indiana is seeking dedicated Nurse Practitioners to join our Post-Acute LTC team. The ideal candidate will possess a strong background in geriatrics, acute care, and chronic disease management, with a focus on high-quality patient care in nursing home and assisted living settings.

You will assess, diagnose, and manage patient care while collaborating with interdisciplinary teams to enhance residents' quality of life, with flexible scheduling and comprehensive benefits.

Qualifications

• Current Nurse Practitioner license in Indiana with certifications in adult, geriatrics or family practice.
• Experience in long-term care settings preferred.
• Proficiency with EMR/EHR systems such as Ethizo, GEHRIMED, Matrix and PCC.
• Knowledge of ICD-9/ICD-10 coding.
• Strong communication and ability to collaborate in a multidisciplinary team.

Responsibilities

• Conduct comprehensive physical examinations and health assessments for patients in long-term care settings.
• Develop and implement individualized care plans based on patient needs and preferences.
• Manage acute pain, behavioral health issues, and chronic conditions through effective treatment strategies.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate and timely medical documentation in HIPAA-compliant EMR/EHR systems.
• Provide education to patients and families regarding health management, disease prevention, and wellness strategies.
• Participate in staff/facility meetings to improve patient outcomes and advance clinical practices.
• Ensure adherence to infection control protocols and best practices in patient care.

Requirements

• Proficiency in EMR systems (Ethizo, GEHRIMED, Matrix, PCC ) for effective medical documentation.
• Knowledge of ICD-9/ICD-10 coding.
• Background in critical care or hospital medicine is a plus; experience in Level I/II trauma centers is desirable.
• Strong communication skills with the ability to work collaboratively within a multidisciplinary team environment.
