Job summary

Samaritan Healthcare & Hospice is seeking a Nurse Practitioner for our Palliative Medicine outpatient team in Atlantic County. The role involves consultation and follow-up visits across outpatient settings, including patients’ homes and clinics, with a focus on building partnerships in Atlantic County.

Responsibilities include health assessments, symptom management, and goals of care conversations in collaboration with families and physicians.

Qualifications

• Prior experience preferred in palliative care, oncology, heart failure, geriatric care, and/or hospice.
• RN, APN license in NJ; DEA and CDS licenses required; Palliative Certification within two years of hire.

• Provides health assessments, diagnoses and treatment for symptom management and goals of care conversations.
• Collaborates with patients, families, and attending physicians to deliver patient- and family-centered care.
• Develops and maintains relationships with internal and external partners to meet patient needs.
• Works with team to coordinate care across Samaritan's programs.
• Full-time, Monday–Friday (40 hours/week) with after-hours call rotation (telephone only).
• Focus on building relationships with partner facilities in Atlantic County.

Nurse Practitioner Palliative Medicine (Outpatient) - Atlantic County

Compensation: $108,000 - $135,000 annually based on years of experience.

Seeking a diligent and compassionate Nurse Practitioner for our Palliative Medicine outpatient team. This NP will work with our Palliative Medical Partners group providing consultation and follow-up visits for patients within outpatient department, with a particular focus in Atlantic County. This includes settings such as the patient’s home, care facilities and clinics. This is a full time position, where the Nurse Practitioner will collaborate with patients, families, and their primary care and consulting physicians to provide excellent patient and family centered care. This role requires skill in establishing and maintaining relationships with new partners.

What You’ll Do:

• Provides a health assessment appropriate to the visit being performed, as well as diagnose and treat patients who require management of symptoms and goals of care conversations.
• Collaborates with patients, families, and the attending physicians and other specialists, to provide excellent patient and family centered care.
• Develop and maintain internal and external relationships, while working with our support staff to meet the needs of both internal and external partners.
• Work alongside other team members to provide coordinated care across all of our Samaritan's family of programs for all patients.
• Full time M-F (40/week) with afterhours call rotation (telephone calls no visits).
• This role will include a focus on building relationships with partner facilities in Atlantic County.
