Job summary

AccentCare, Inc. is seeking a Nurse Practitioner with a focus on Palliative Care to join our full-time team in North Pinellas, FL.

This on-site role covers local care delivery with a strong emphasis on developing individualized care plans and symptom management for patients with chronic and terminal illnesses. The position requires NP licensure in Florida, a valid DEA, and CPR certification; travel to different locations is expected.

Qualifications

  • Licensed to practice as a Nurse Practitioner in the state of operation (Florida).
  • One year experience as a Nurse Practitioner is preferred.
  • Active DEA license in the state where care is being provided.
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred.
  • Must be a licensed driver who can travel to all business locations.

Responsibilities

  • Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
  • Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
  • Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge planning; and patient/family teaching.
  • Maintain communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis.
  • Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.
  • May provide support at time of death.
  • Prescribe medication as permitted by the state and/or the State Board of Nursing.
  • Participate in continuum of care for patients that are hospice appropriate and ongoing education programs.

If you meet these qualifications, we would love to meet you:

  • Graduated from an approved school of professional nursing
  • One year experience as a Nurse Practitioner is preferred
  • Licensed to practice as a Nurse Practitioner in the state of agency operation
